Clinical Note Finalization and Linked Amendment Evidence
Recorded retrospectively on October 28, 2026 from retained SSD evidence. The later pre-unlock VaultStore snapshot contains the note lifecycle implementation: session-linked draft creation, explicit finalization, rejection of stale editors and non-draft edits, linked amendments with a reason, and rollback when persistence fails. Finalizing an original session note marks its source session complete. Amendment creation preserves the original record and reuses an existing amendment instead of creating competing revisions.
The October 14 note-lifecycle-final-tests.log records 126 tests passing with zero failures. Named tests cover amendment history, failed-finalization rollback and retry, legacy decoding without invented finalization, idempotent session review and reload, stale-editor rejection, and refusal to create a clinical record from an unlinked session. These cases also passed in the October 15 full log.
This supersedes the earlier roadmap statement that formal note finalization and amendment remained entirely unimplemented. It does not establish a legal electronic-signature system or production readiness. A fresh build and rendered UI verification have not been performed during this audit. The retained source files are partial snapshots, not a verified complete October 14 source commit.
Decisions
- Preserve historical entries and publish explicit corrections.
- Distinguish historical test results from fresh validation.
- Treat incomplete source preservation as an open item.
Next Actions
- Recover and checkpoint the complete later source before claiming reproducibility.
- Resume from the archive current-state index and verify referenced evidence.
